The Affordable Care Act and the advent of Electronic Health Records are encouraging an increasing level of electronic population health management in clinical care. These trends are leading to a new class of software systems and EHR add-on modules that are loosely defined as “population health management systems.” What specifically do population health management systems do? How are they architected to support population health needs? How might they support public health?
